			Missions progress the main story in Final Fantasy XI. You will start off with your chosen Nation's main story, which can be initiated from a gate guard. Doing these will increase your Nation Rank (not to be confused with Fame/Reputation - see next question for explanation of that).

			Each expansion then comes with its own set of story missions. From level 30 you can progress with Chains of Promathia missions. And after reaching rank 6 with your nation, you will also be able to complete the Rise of the Zilart story missions.

			Quests are essentially side content, and can serve a number of uses, such as raising your fame/reputation with various factions, getting unique gear, gil, maps, or unique perks (for example, chocobo license, airship pass, etc).

			Fame is used for a few different things. It slightly decreases the cost of items from npcs in that area, and slightly increases the amount that those npcs will give you for items you sell to them. Having higher level fame is also a prerequisite for certain quests.

			Generally, each faction will have at least one repeatable quest you can complete to easily level up your fame. 			No, gear does not sync when in level sync parties. As such, even when you obtain a gear upgrade, you may want to hang onto the old gear for a few levels in case you join a level sync party and can't use your new gear.
